The Enhanced Pathway provides evidence-based and therapeutic interventions for people with complex mental health needs. Our teams work with service users and their families and carers to enable them to live well in their communities.

We want to make sure our staff have the right knowledge and skills to deliver safe, modern and transformational interventions, so that people with mental health difficulties are treated with dignity and respect and receive the care and support they deserve.

You will be fully supported by a dedicated team to undertake this complex and demanding role.

What you will do...

You will be responsible for the co-ordination of care delivery within our integrated team of health and social care staff, which will involve you undertaking holistic assessments, care planning and delivering treatment interventions. You will be expected to provide support and a preventative approach to service users to avoid hospital admission. You will undertake MHA assessments on service users within the community; you will also hold a caseload.

You will provide an important and valued professional function working with some of the most vulnerable people in society. Your professional expertise and approach will ensure individual needs and aspirations are supported at all times, risk is managed proportionately and safeguarding is of paramount importance. You can expect commitment to your personal and career development.

What you will need...

You will need to be compliant with the Care Act, Mental Health Act, Human Rights Act, Mental Capacity Act and associated Codes of Practice to fulfil your duties in this role.

You will need to have experience of working with the service user group and be able to use social work methods to promote positive change, independence and wellbeing in accordance with assessed need, whilst promoting choice and control for adults and carers.

Working knowledge of relevant legislation and ability to apply legislation to practice is required and good IT skills to support accurate and timely case recordings.

You must hold a Social Work England recognised qualification, and have evidence that you are registered with Social Work England.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.  It is essential that you hold a full and valid driving license and it is expected that you will either use a council vehicle or your own car.

Appointments at Senior Practitioner Level will be subject to meeting progression requirements.

Appointments to qualified AMHP will be subject to qualification.
