An exciting opportunity has arisen to join our Early Onset Dementia Team based at Folly Hall in Huddersfield. If you are a qualified AMHP you will be expected to support the AMHP hub to undertake AMHP assessments as part of your role.

The early onset dementia team provides a specialist service for people aged 18-64 who have a diagnosis of dementia or are suspected of suffering from a type of dementia.  The team was developed to provide a multi-agency approach to meet the needs of younger people with dementia, their carers and families.  We have a mix of mental health nurses and social care professionals with access to occupational therapy, dietetics and psychology.

At Folly Hall we are an agile workforce which means you will be provided with the necessary equipment to allow you to work from a base of your choice and from home as necessary. The team are very supportive and meet weekly to discuss any complex cases. This enables joint decision making and care-planning. You will need to be able to work autonomously as well as within a team and be able to assess risks for people with dementia.

You must hold a HCPC recognised Social Work Qualification and be registered with the SWE.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.  It is essential that you hold a full and valid driving licence and it is expected that you will either use a council vehicle or your own car.
