The Team

There is a really supportive feel to our team; we have approachable and supportive managers who have an open-door policy and are willing to listen. The team all pull together to support each other and share their expertise. No two days are ever the same, but when the going gets tough we are all there for each other.

The Role

You will work with a wide range of customers and their carers to ensure they receive the right input at the right time. Supporting customers to maintain their well-being and independence through personalised support plans.

You will receive referrals from the Hospital Assessment Team, Gateway to Care and the Community Well-being Hubs as part of the Self-Directed Support Pathway.

Part of your role will be to offer timely advice and information to customers and their carers so they can make an informed decision regarding their care and support needs.  You will then facilitate and arrange support to meet their specific outcomes through a variety of options such as direct payments, commissioned or independent care providers and community organisations

The pace of work means you need to be organised and resilient, but most of all you will care about the people we are working with.
