Job 1 - South Kirklees - You will work 37 hours per week 5 days over 7 between 6am and 11pm, including weekends, evenings and Public Holidays.

Job 2 - North Kirklees - You will work 18.5 hours - 3 days over 7 with current working pattern of Weds, Thurs, Fri.  This could be subject to change.

You will work within a supportive and experienced team of people dedicated to helping adults maintain their health and wellbeing. 

You will manage a team providing short term urgent support with a key outcome of re-ablement. You will co-ordinate your team effectively and support them in visiting and providing a quality service to our service users. 

The primary focus of your role is to ensure the provision of quality support to service users, exceeding Care Quality Commission expectations and standards.  You will also establish and maintain relationships with service users, relatives and other professionals. 

You will be passionate about developing your team and providing ongoing training, taking feedback from service users and ensuring the effectiveness of the service. 

Requirements for this job:

  • High level of understanding of Adult Social Care and knowledge of re-ablement.
  • Possession of NVQ 3 in care or relevant qualification or willing to undergo such training.
  • IT skills and experience with computer packages e.g. Microsoft Word, Access and Excel.
  • Understanding of relevant legislation, guidance and Government agendas.
  • Ability and willingness to work flexibly. This may include shift working to meet the needs of the service.
