Moorlands Grange is seeking a dynamic and dedicated Team Co-ordinator to join our close-knit community where we believe in enhancing lives and creating a supportive environment for both residents and our exceptional team.

Job Overview

At Moorlands Grange, we provide short-term stays lasting 4-6 weeks in a residential setting to prevent physical decline and improve overall wellbeing. Our purpose? To support individuals who are ready to be discharged from hospital or those who may not have access to crucial rehabilitation and reablement support in their homes. As a Team Co-ordinator at Moorlands Grange, you'll play a key role in shaping the care experience for our residents and supporting our committed team of care professionals.

 

Key Responsibilities:

  • Lead and motivate a team of Support Workers, fostering a positive and collaborative work environment.
  • Ensure the delivery of high-quality, person-centred care to residents, meeting their individual needs and preferences.
  • Collaborate with the multi-disciplinary team to develop and implement care plans, ensuring they align with regulatory standards.
  • Provide guidance, support, and training to team members, promoting continuous professional development.
  • Monitor and evaluate care practices, identifying opportunities for improvement and innovation.
  • Act as a liaison between residents, their families, and the wider care team.

 

What We’re Looking For:

  • Demonstrable experience or ability to lead a team within a care setting.
  • NVQ Level 3 in Health and Social Care or equivalent qualification or be willing to work towards.
  • Strong communication and interpersonal skills, with the ability to inspire and motivate a team.
  • A compassionate and person-centred approach to care leadership.
  • Knowledge of relevant regulations and standards in the care sector.
