Social Worker – Mental Health

South Kirklees Enhanced Team is looking to recruit an enthusiastic and motivated individual to join our existing multi-disciplinary team as a Mental Health Social Worker.
The South West Yorkshire Foundation Trust has recently transformed its Acute and Community services. As part of the transformation process the Enhanced Pathway provides services for people with complex mental health needs in the community.
The Enhanced Pathway provides a Flexible Assertive Community Treatment (FACT) model of care 5 days a week.
You will be expected to attend the daily morning meeting to feedback risk situation and to formulate care/risk assessment/management plans.
The service covers Monday to Friday 9am to 5pm. You will be responsible for undertaking community assessments and delivering treatment interventions as well as triage of referrals and be responsible for assessments, care-planning and co-ordination of care delivery within a team approach.
You should be forward-thinking, proactive, and the ability to form effective professional working relationships with team members and service users is an essential requirement of the job, as is effective time management.
Awareness of working practices, legal frameworks and changing agendas affecting service users is also required.
You will be responsible for assessing the mental health needs of people referred into the service and formulating with them and their carers an appropriate plan of support. You will be required to make eligibility determinations as defined in the Care Act (2014). You will be expected to understand the legislation around the Mental Health Act (1983, amended in 2007) and the Mental Capacity Act (2005) as well as other legal frameworks which affect service users.
There will be a strong focus upon your own personal professional development to enhance your skills and practice. Our aim is to develop confident and competent practitioners, thereby supporting better outcomes for people we work with and equipping you with real and transferable skills for the future.
You must hold a Social Work England recognised professional social work qualification and have evidence that you are registered with Social Work England.
Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving licence and it is expected that you will either use a council vehicle or your own car.
