This role is based in the South short term and urgent support team. The team is part of Intermediate care services within Kirklees. (KILT) Kirklees Independent living Team

There’s a really supportive feel to our team; we have approachable and supportive managers who have an open door, regular supervision, and are willing to listen; we’re an experienced team, we all pull together to support each other and share our expertise. Intermediate care and the short term and urgent support service is fast paced, no two days are ever the same.

You will work as part of the team and be able to self-allocate work as required. The focus of your role will be to work with service users once their period of re-ablment is completed ensuring that future identified care and support needs are met.

You will work closely with Locality managers, brokerage, care Nav’s and support options.

The pace of work means you need to be organised and resilient but most of all you’ll care about the people we’re working with.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.  It is essential that you hold a full and valid driving licence, and it is expected that you will either use a council vehicle or your own car (exceptions can be made for disabled applicants).

Requirements for this job:

  • Experience of working in a caring environment in a paid or unpaid capacity with a relevant service group.
  • An understanding of legislation, procedures etc. relating to the relevant service user group.
  • Ability to manage own caseload and allocation
  • High level of communication and negotiation skills
  • Ability to demonstrate literacy, numeracy and computer skills to complete assessments and correspondence, complete financial statements and keep accurate records.
  • IT skills to be able to maintain accurate electronic records.
  • An understanding of the differing physical and emotional needs pertaining to the specific user group.
  • Ability to identify social care needs of individuals and formulate support plans
  • Ability to prioritise and meet deadlines
  • Understanding of other agencies and their contribution to service delivery
  • Willingness to continue further personal training and development
