The Service operates from 8.00am to 8.00pm across 7 days a week; you will work on a rota basis including weekends and Public Holidays for which any relevant enhancements will be paid.

Through a joint Health and Social Care approach to targeting patients in Accident & Emergency (A&E) Departments more people will have their acute health and social care needs managed within primary care and will avoid emergency and non-elective admissions by the provision of interim Health and Social Care Services at the point of entry.

The Service offers a quick response which will lead to people being returned home with support tailored to their needs and where necessary, alternative accommodation being offered rather than an acute hospital bed. You will be working with people who attend the Accident and Emergency Department or Medical Assessment Unit who after medical assessment do not need admittance but there are significant obstacles for their return home.

We are looking for enthusiastic, flexible and confident people who have some experience of working in a caring environment with vulnerable people. Your roles will be to carry out a low level assessment of patients and you will therefore need an understanding of the needs of vulnerable people. You may also be required to provide equipment such as assistive technology and or enablement services to assist them to return home. You will be responsible for the installation/delivery of the equipment and possible transportation of the patient if appropriate.

 Requirements for this job:

  • Experience of working in a caring environment in a paid or unpaid capacity with a relevant service group.
  • Ability to demonstrate literacy and numeracy skills to write assessments and correspondence, complete financial statements and keep accurate records.
  • Awareness of, and commitment to, implementing Health and Safety requirements at work.
  • An understanding of the differing physical and emotional needs pertaining to the specific user group.
  • Ability to identify needs of individuals and formulate care plans.
  • Ability to prioritise and meet deadlines.
  • Understanding of other agencies and their contribution to service delivery.
  • Willingness to continue further personal training and development and to agree a personal development plan.
  • Possession of a full and valid driving licence (exceptions can be made for disabled applicants).

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving licence and it is expected that you will either use a Council vehicle or your own car.
