We are looking to recruit to a 37-hour Care Navigator Post. The role will involve working with a wide range of customers and their carers to ensure they receive the right input at the right time and are directed to the right place. Supporting customers to maintain their wellbeing and independence utilising a strengths-based approach.

Whilst you will primarily work out in the community, you may also be expected to work on occasion within Dewsbury District Hospital Assessment Team, or Huddersfield Royal Infirmary Assessment Team. Where you will facilitate the safe and timely discharge of individuals from hospital back into the community. During this time, you will form part of the multi-disciplinary ward-based team to promote discharge and reduce the length of stay for patients

You will receive referrals from Gateway to Care, Partner Agencies and the Wellbeing Assessment Hubs as part of the Self-Directed Support Pathway. Care Navigators work with customers to enable them to remain independent in their own homes and local communities. Through appropriate sign posting, timely advice, sourcing and arranging support to meet their specific needs.
