It is an exciting time to come join us in Adult Integrated Care. Using a strengths based approach to care, our vision is to promote independence and meeting the health and wellbeing needs of vulnerable people.

We are looking to recruit an enthusiastic and hardworking, Social Worker, within our Community team for learning disabilities East in the Wakefield District. You must be able to provide effective support for people with learning disabilities, using a strength based approach to enable individuals to live as independently as possible with more choice and control.

You will work closely with our key partners in the Intensive Support Team, to carry out person led assessments with people aged 18 and over with a learning disability. Based on strengths to promote positive change, self-determination and to prevent harm to people with an appearance of social care needs, in order for people to achieve their personal outcomes.

The team consists of social workers, care co-ordinators and co-ordination support officers. As an experienced post qualified Social Worker, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am – 5pm and to 4.30pm on a Friday.

As a team player, you will need to demonstrate an ability to, work under pressure, be well organised and able to effectively prioritise and risk assess/manage a complex caseload. You will provide access to expert knowledge, application and professional advice, to ensure legal compliance. You will have experience, knowledge and commitment to strength-based assessments and support planning. You will be able to undertake complex tasks, such as completing joint section 117 after care planning, chairing significant meetings, offering expert support and advice to case conferences, producing high quality and more specialised assessments and reports for a range of functions including court. The ability to travel is essential.

Applicants for this role must be qualified to a Degree level or equivalent and be registered as a Social Worker with Social Work England, with essential post qualifying relevant training, qualification.
