This job is internally restricted. To apply for this job you must be an employee of the Council for the duration of the recruitment process – up to and including the start date. We are looking to recruit to a 37 hour Care Navigator post. The successful candidate will work closely with the Huddersfield Royal Infirmary Frailty Team and A&E staff to avoid hospital admissions and reducing hospital length of stay. You will support people to return home as a first resort and to maintain their independence with creative support packages. This might include assistive technology/other pieces of equipment, providing timely information/advice or sourcing assessed services and alternative community provision.

Whilst you will be primarily based in Huddersfield Royal Infirmary, you may be expected to work out in the community, or from home on occasion.

There is an expectation that you will carry a small caseload. You will also be expected to keep a record of key performance indicators and report on the success of the role.

Individually tailored induction will be offered to the successful candidate.
