This role is an exciting opportunity to join the Kirklees Peripatetic Team primarily covering North and South Kirklees care homes in an extremely flexible and supportive environment.

Team Details:

In the Peripatetic team, you will primarily be working with adults living within a 24-hour care home setting who have social care needs of varying complexity. You will be required to complete holistic assessments and reviews of a person’s needs to ensure they are supported and to achieve the best outcomes. This will involve building professional relationships with care home staff, seeking and encouraging the contribution of the person, their carers and relatives, as well as others involved in their care. As part of the role, you will engage in multi-agency working with a wide range of health and social care professionals including care homes and health professionals.

There’s a really supportive feel to our team with approachable managers and assessors who have an open-door policy and are willing to listen; we all pull together to support each other and share our expertise.

Requirements for the role:

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.  It is essential that you hold a full and valid driving licence, and it is expected that you will either use a council vehicle or your own car (exceptions can be made for disabled applicants).
