Working to the community discharge to assess model, the short term and urgent support teams provide support to people enabling them to remain or return home following an acute episode or a sudden deterioration in their health
The team is part of Intermediate care services within Kirklees. (KILT) Kirklees Independent living Team. There’s a really supportive feel to our team; we have approachable and supportive managers who have an open door, regular supervision, and are willing to listen; we’re an experienced team, we all pull together to support each other and share our expertise.
Intermediate care and the short term and urgent support service is fast paced, no two days are ever the same
You will work as part of the team and be able to self-allocate work as required. The focus of your role will be to work with service users and health and social care professionals ensuring that future identified care and support needs are met. You will work closely with Locality managers, brokerage, care Navigation and support options
The pace of work means you need to be organised and resilient but most of all you’ll care about the people we’re working with. We are committed to providing our Senior Community Assessment and Support Officer (SCASO) with exceptional levels of support. We understand that working within social care isn’t an easy job and how important it is to take care of ourselves. Requirements for this
If you are internal and want to apply for this job as a secondment you must be a substantive employee of the Council with over 6 months continuous service and have successfully completed your probation period.
